Professional, individualized support to help clients and families navigate care at home and beyond.
- Wellness assessments
- Vital-sign monitoring (including blood pressure, pulse oximetry, temperature, weight, and blood glucose)
- Medication organization, reminders, and pharmacy refill coordination
- Care coordination with physicians and specialists, including follow-up appointment scheduling
- Accompaniment to medical appointments and appointment advocacy
- Post-hospital discharge support
- Health education for clients and families
- Education on chronic conditions and management, including congestive heart failure (CHF) and diabetes
- Postoperative incision and recovery monitoring
- Respite care
- Caregiver education and support
- Home-safety and fall-risk assessments
Clinical Services
With a valid provider order, when required
Clinical nursing services provided within applicable professional scope, licensure, and order requirements.
- Mobile blood draws
- Wound care
- Foley catheter insertion, irrigation, and maintenance
- IV infusion administration and vascular-access device maintenance and monitoring
- Tube-feeding administration
- Colostomy, ileostomy, and urostomy care
